We offer a continuum of residential services, including medically-monitored detoxification and support across levels of care defined by the ASAM model. Our goal is to support each individual’s unique journey through group and individual counseling, holistic programming, educational workshops, and personalized treatment planning.

We provide services across four ASAM levels at our facility:

  • 3.1 – Clinically Managed Low-Intensity Residential Services
  • 3.2 – Clinically Managed Residential Services
  • 3.3 – Clinically Managed Population-Specific High-Intensity Residential Services
  • 3.5 – Clinically Managed High-Intensity Residential Services

Clients are assigned to one of these levels and work closely with their treatment team to complete individualized programming, therapy and ancillary activities geared towards their level of acuity and recovery goals. Levels are adjusted as clinically indicated based on ongoing evaluation and an individual’s responsiveness to treatment. This multi-level, outcomes-focused approach aligned with ASAM standards ensures clients receive the precisely tailored care they need for lasting recovery

Within our comfortable, home-like setting featuring 18 beds across three houses, clients can focus fully on healing mind, body and spirit. We treat patients dually diagnosed with co-occurring mental health or medical needs, understanding that full wellness requires addressing the whole person.

McMahon

Dr. Angela McMahon has a doctorate in Educational Counseling Psychology and has worked in compliance for over ten years. She aligns herself with the most ethical and compassionate programs, so this was a natural blend between her and 5 Five Star. She really believes in the team and the vision to make a difference in people’s lives and looks forward to longevity with a program that has the same beliefs in empowering others to live their best lives in recovery

Dr. Julio
Meza

Addiction and Family Medicine Physician

Julio Meza, MD, is an Addiction and Family Medicine Physician at the David Geffen School of Medicine at UCLA and a Psychiatry in Primary Care Certified Physician at UC Irvine.

Dr. Meza was born and raised in El Salvador. He graduated Cum Laude from Universidad Evangelica de El Salvador. After graduating from medical school, he was accepted into a surgical residency and completed an internship at the Hospital Militar Central in El Salvador. He left El Salvador to pursue his dream of becoming a U.S. licensed family physician.

While in Los Angeles, Dr. Meza worked as a medical assistant instructor and program director, training students to work in the areas of San Fernando, North Hollywood and Los Angeles then joined UCLA and completed his residency in Family Medicine and also Fellowship in Addiction Medicine. Dr. Meza enjoys working with patients from all backgrounds and provides nonjudgmental compassionate care.

Shaza is a board certified nurse practitioner with broad experience as an addiction treatment provider. Her previous experience includes work as a critical care registered nurse throughout Southern California and as a provider treatment programs. She is a graduate of UC San Diego, the University of Oklahoma, and Azusa Pacific University. She is particularly interested in the study of addiction as a maladaptive response to universal human suffering.

I am extremely passionate about the field of Nutrition. Being a Registered Dietitian for the past 15 years has been absolutely amazing. Making a difference in people’s lives and achieving their desired outcomes is very rewarding. I am not just talking about weight loss. I am referring to changes that people make in their relationship to health, whether it’s joining the gym or doing yoga, eliminating alcohol or sugar from their lifestyle and so on.

My goal is to help people incorporate healthy eating habits and heighten their awareness of the importance of health and nutrition.

I find satisfaction in knowing that I helped change someone’s life in a positive way. There is nothing better than watching a patient’s health and well-being improve as a result of implementing a healthy lifestyle. I believe that education is the most critical element of disease prevention and treatment.

RoseAnn Kelley, C.Ht. is a Certified Hypnotherapist, Behavioral Interventionist, Instructor, Writer, and Inspirational Speaker, with over 26 years of experience in the field of Behavior Modification and Habit Reconditioning.

RoseAnn holds a Bachelor’s Degree in Journalism and English from The Ohio State University, as well as a Certification and Diploma in Clinical Hypnotherapy from Hypnosis Motivation Institute (HMI), the nation’s first accredited College of Hypnotherapy.

Graduating at the top of her class at HMI in 1998, she accepted an invitation to join their clinical program as a staff Hypnotherapist, resident Mentor and Instructor of Basic Hypnosis 101, Handwriting Analysis, Past Life Regression and various practicum workshops.

Growing up with parents who struggled with alcoholism, prescription drug addiction and sexual abuse, RoseAnn saw firsthand the difficulty of overcoming addictions, codependency and trauma. This led her to specialize in sustained recovery from substance use disorder, anxiety, depression, and inner child healing.

RoseAnn’s therapeutic modalities include hypnotherapy, neurolinguistic programming, dream therapy, guided imagery, and Quantum Touch Healing Technique to bring the Conscious and Subconscious Minds into alignment and create Heart Brain Coherence. This addresses underlying issues to free us from inner conflict, self-sabotage and self-destructive behaviors, so we can achieve our goals and live a healthy life full of love, happiness, peace and prosperity.

RoseAnn’s wholistic approach to Mind, Body & Spirit wellness is a natural complement with the mission of 5 Star Care LA, to provide personalized treatments plans, holistic programming, and educational workshops with dignity, respect and support for each individual’s unique journey to healing. A focus of Ari’s profession surrounds the drug and alcohol treatment field. He has extensive experience and knowledge surrounding recovery. Another focus of Ari’s practice is couples and relationships. He has worked for a number of years with many individuals and couples experiencing difficulties in relationships.
